To access these forms/information sheets you need to have access to MS Word.

USING THE SELF REFERRAL FORM 

If you complete the Self Referral Form please save it to your PC and send it to us as an attachement to an e-mail, it would be helpful if you could give us an idea of preferred days/times of day for an appointment in your e-mail.

We will then contact you by phone to arrange a suitable appointment.

SOLICITOR REFERRAL FORM - to be used by solicitors only

The "Solicitor Referral Form" listed is for the use of solicitors only. Members of the public who wish to refer themselves to the service should use the "Self Referral Form"

COURT ORDERED CONTACT ACTIVITY REFERRAL FORM - to be used by CAFCASS/COURT

This form is for referral to Mediation Assessment (Information) Meetings or Separated Parents Information Programme (PIP) where parents have been ordered by Court to attend an activity.
